The AcuityModel™

Five pillars. One coordinated system of care.

The AcuityModel™ is the framework that guides how we design, deliver, and improve care across every division. Nothing falls through the cracks.

1

Person-Centered Planning

Every plan starts with the individual's goals, preferences, and strengths. Not a diagnosis.

2

Coordinated Case Management

Every consumer is supported by a licensed social worker who coordinates across all services and providers, keeping the plan connected and the care consistent.

3

Research-Grounded Supports

Behavioral, clinical, and community supports grounded in current research and best practices.

4

Workforce Excellence

The AcuityCare Academy trains and retains a qualified, compassionate direct support workforce.

5

Continuous Quality Improvement

We track outcomes, ask hard questions, and adjust when something isn't working. Quality is a practice, not a checkbox.
